This fact sheet provides an overview of the Support at Home Assistive Technology and Home Modifications (AT-HM) scheme, which will give Support at Home participants access to products, equipment and home modifications based on their assessed needs.
AT-HM scheme data collection
These resources can be used to guide requests for AT-HM scheme funding for Support at Home participants who moved from the Home Care Packages Program. The AT-HM scheme data collection closed on 13 February 2026. Requests submitted before this date will continue to be processed.
