Schedule
Stoma Appliance Scheme
The Stoma Appliance Scheme provides free stoma appliances to people who have a stoma (ostomates), through their stoma associations. All the items subsidised under the scheme are listed on the Stoma Appliance Scheme Schedule.
Stoma Appliance Scheme schedule
The Stoma Appliance Scheme schedule lists all of the stoma-related products and appliances that we subsidise under the scheme. You can view the full list, or just the additions, deletions or variations.

Stoma Appliance Scheme – Application form
Use this form to apply to join a stoma association to access products on the Stoma Appliance Scheme.
Stoma Appliance Scheme – Application form for additional supplies – Clinical
Use this form when a Stoma Appliance Scheme participant wishes to access additional supplies for clinical reasons.
Stoma Appliance Scheme – Application form for additional supplies – Travel or remote location
Use this form when a Stoma Appliance Scheme participant wishes to access additional supplies if they are travelling overseas or living/working in a remote location.
Stoma Appliance Scheme – Cleanser wipe authorisation form
Use this form to apply for authorisation to access cleanser wipe products under the Stoma Appliance Scheme.
Stoma Appliance Scheme – Deodorant and absorption gelling sachets authorisation form
Authorised health professionals can sign this form to authorise a patient to order deodorant and absorption gelling sachets from their stoma association. The patient's needs should be reviewed again after 6 months.
Stoma Appliance Scheme – Tiemann tip catheter authorisation form
Authorised health professionals can sign this form to authorise a patient to order Tiemann tip catheters from their stoma association.
Stoma Appliance Scheme – Irrigation kit authorisation form
Stomal therapy nurses should sign this form to authorise a patient who requires an irrigation kit to order it from their stoma association.
Stoma Appliance Scheme – Peristeen Plus authorisation form
Use this form to apply for access to Peristeen Plus on the Stoma Appliance Scheme. This form should be completed by the applicant or their authorised representative as well as the applicant’s authorised health professional.

Suppliers of stoma-related products should complete this form to list a new product on the Stoma Appliance Scheme Schedule, in benchmark groups 1 to 7.

Suppliers of stoma-related products should complete this form to list a new product on the Stoma Appliance Scheme Schedule, in groups 8 to 11.
Stoma Appliance Scheme – Application to list products on the schedule (price premium products)
This form should be used by sponsors of stoma-related products seeking to add a listing to the Stoma Appliance Scheme (SAS) Schedule, or to add an additional variant to an existing SAS code and product range listed on the SAS Schedule.

Suppliers of stoma-related products should complete this form to vary a product listed on the Stoma Appliance Scheme Schedule.

Suppliers of stoma-related products should complete this form to delete a product listed on the Stoma Appliance Scheme Schedule
Stoma Appliance Scheme product features table to add additional variants to Benchmark Group 1-7 or Price Premium Products
Suppliers of stoma-related products should use the Microsoft Excel Product Features Table to add additional variants within the same product range to an ‘Application to list a product – Benchmark Groups 1-7’ or ‘Application to list a product – Price Premium Products’.
