These Guidelines are a general guide to assist a pharmacist making a request to the Minister for approval to supply pharmaceutical benefits at particular pharmacy premises.

As permission holder under section 91/91B of the National Health Act 1953, you must complete and submit this form to authorise a pharmacist(s) to sign pharmaceutical benefit claim forms and endorse pharmaceutical benefit prescriptions on your behalf.
