What you need to know
If you are an owner of an approved pharmacy under section 90 of the National Health Act 1953, you can authorise another person to make enquiries and/or sign PBS claim forms on your behalf.
To assign this authority you must provide us with the details of each person working at the approved pharmacy who you permit to make enquiries or claims on your behalf.
You must also remove any person from our record of your approved pharmacy who no longer requires the authority to make enquiries and/or sign PBS claim forms.
A request to assign authority to another person will displace any previous request.
Please provide the details of every person who you want to assign authority to even if you have provided these details before.
Only the owner(s) of an approved pharmacy can request changes to the record of an approved pharmacy.
What you need to do
To authorise another person to make enquiries and/or sign PBS claim forms on your behalf:
- Complete the Authority to permit person(s) to enquire and/or sign claim forms on behalf of approved pharmacist(s) form.
- Submit the form and any related documents through the PBS Approved Suppliers Portal – you must be signed in to submit the form.
