We have published the forecast opportunity for the upcoming Chronic Conditions Prevention and Integrated Care Program grants round on GrantsConnect.
This grant opportunity aims to improve health outcomes for people in Australia living with, or at risk of, chronic conditions by supporting early detection and effective management.
The government is investing $109.9 million over 3 years from 2026–27, with ongoing funding of $38.3 million per year.
Through a competitive process funding will be directed towards initiatives that show measurable, sustainable improvements in chronic condition outcomes
Guided by the National Strategic Framework for Chronic Conditions 2026–2035, the program will invest strategically and competitively to ensure transparency, fairness and alignment with government priorities.
The objectives, aligned to the framework, are:
- Health promotion and education
- Embedding prevention and early detection
- System-integrated, multidisciplinary, person-centred care
- Managing multimorbidity
- Enhanced and targeted health equity support.
To assist prospective applicants, we have also made available draft Grant Opportunity Guidelines and Frequently Asked Questions (FAQs).
The guidelines are subject to change and a final version will be published when the grant opportunity is released.
The FAQs provide guidance on eligibility requirements, application procedures, assessment criteria and recommendations for preparing proposals.
Applicants are advised to review these materials thoroughly, as they contain relevant information regarding department expectations and approaches to optimising application success.
