We have developed a video for providers to share with older people, their families and carers about Support at Home’s short-term pathways.
Support at Home helps older people live at home for longer. It provides ongoing services for regular care and 3 short-term pathways for extra support when needs change.
The video:
- explains how the short‑term pathways work and when they may be helpful
- helps older people make informed decisions about their aged care
- supports meaningful conversations between older people, their families, carers and health professionals.
Watch and share the video with the older people you support (also available in variety of languages, including Auslan).
