  • The Multi-Purpose Service Program

    The Multi-Purpose Service Program (MPSP) delivers integrated health and aged care in rural and remote Australia, helping older people stay close to family and friends through flexible, community-based services tailored to local needs.
  • The National Sign Language Program (NSLP)

  • The National Workforce Centre for Child Mental Health

    The National Workforce Centre for Child Mental Health, delivered by Emerging Minds, assists professionals and organisations who work with children and their parents or families to have the skills to identify, assess and support children at risk of mental health conditions.
  • Thriving Kids

    Thriving Kids is a national system of supports for children aged 8 and under with developmental delay and/or autism with low to moderate support needs, and their families, carers and kin.

  • Transition Care Program

    The Transition Care Program helps older people recover after a hospital stay by providing short-term care for up to 12 weeks. Read about the program, care provided, and why it’s important.
  • Transition to Practice Program

    The Transition to Practice Program provides an evidence-based framework of support over 12 months to nurses who have transitioned to primary health care and feel they would benefit from additional support. We fund the Australian Primary Health Care Nurses Association (APNA) to provide this program.
