Thank you for submitting this information. The Australian Government Department of Health (the Department) is collecting your personal information for the purposes of investigating, assessing and responding to fraud or suspicious activity by health providers against the Australian Government’s health related programmes.

The Department of Health can be contacted by telephone on (02) 6289 1555 or free-call 1800 020 103 or by using the online enquiries form at the Department's website.

You do not need to provide your personal details. You can use a made up name or remain anonymous, but it may be helpful if the Department can contact you to ask for more information and it may assist the Department in investigating your tip-off.

However, if you provide your personal details, you are consenting to the Department collecting:

    • sensitive information about you, including health information, and
    • information from third parties, such as police and other government agencies, including the Australian Health Practitioner Regulation Agency, if necessary
for the purposes indicated above.

The Department may disclose information provided by you, including your contact details, to other government agencies e.g. the Australian Federal Police.

The Department has an Australian Privacy Principle (APP) privacy policy which you can read at the Privacy Policy page. You can obtain a copy of the APP privacy policy by contacting the Department using the contact details set out above. The APP privacy policy contains information about:
    • how you may access the personal information the Department holds about you and how you can seek correction of it; and
    • how you may complain about a breach of the Australian Privacy Principles
The Department is unlikely to disclose your personal information to overseas recipients.
