Reporting to the register
As a health professional you play a key role in reporting data to the register.
Information about participants and their results is used to make sure that:
- participants with positive results move through the appropriate screening pathway
- health professionals give appropriate clinical advice and offer the right services, treatment and care.
Payments for information
Health professionals can get paid for correctly completed forms sent to the register about participants who've had a positive immunochemical faecal occult blood test (iFOBT) result through the National Bowel Cancer Screening Program. Forms are listed below.
- $14.30 (including GST) for each Colonoscopy Report or Histopathology Report
- $7.70 (including GST) for each GP Assessment Form or Adverse Outcomes Report.

Changes to payments
The register is introducing a portal for providers and will also integrate with selected clinical management software.
After this is released towards the end of 2020, there will be a new process for reporting participant information to the register and payments will end.
You can keep submitting forms until we advise you that payments are ending.
You can submit participant information to the register using these forms. You can submit online using the SmartForm links or follow the instructions in the form descriptions to submit as a PDF.
Colonoscopists use this form to report an adverse outcome for a procedure relating to diagnostic investigation of a National Bowel Cancer Screening Program participant who has received a positive iFOBT result. Only use this form if this has not already been included in the Colonoscopy Report.
General practitioners (GPs) use this form to provide information about consultations with bowel cancer screening patients who have received a positive iFOBT test result.
Histopathologists use this form to report the results for specimens collected during a surgical resection procedure for a National Bowel Cancer Screening Program patient with a positive iFOBT result.
Colonoscopists use this form to report adverse outcomes of diagnostic investigation procedures for National Bowel Cancer Screening Program patients who have received a positive iFOBT result. Only use this form if the information has not already been included in the colonoscopy and histopathology.
