Because of this, it is likely that no one doctor or healthcare practitioner has a full picture or understanding of your health and medical history. A PCEHR will help bring a number of your health records together, reducing the need to repeat information unnecessarily when you are unwell or when you are caring for someone.
The PCEHR system is about improving your health care by giving your healthcare providers the right information at the right time.
If you chose to have and use a PCEHR, your key health information will be accessible online to whoever you authorise. The system will allow your key health information to travel with you through the health care system. Once the PCEHR system is fully implemented, your health information will be available to support your ‘end-to-end’ health care needs regardless of where and when you need it; even with a new healthcare provider.
Other benefits of using a PCEHR includes the ability to improve continuity and coordination of care across a range of healthcare providers – leading to better informed decision making and more effective management of your medications leading to a reduction in avoidable medication-related adverse events.
