This review is a follow up and extension of a 2004 report commissioned by the New South Wales Department of Disability Aging and Home Care (DADHC) from The Centre for Developmental Disability Studies (CDDS) at the University of Sydney.The authors would like to acknowledge the contribution of the following people to this review: David Trembath, research assistant, who assisted in the preparation of all aspects of this review.
