Chapter 1: About the Guidelines
These Guidelines are intended to provide general information about the Transition Care Program. The Guidelines are linked to the Aged Care Act 1997 through a Transition Care Payment Agreement between the Australian Government and each state or territory government represented by their health department. Compliance with the Guidelines is a requirement under the Transition Care Payment Agreement.
The Guidelines should be read in conjunction with the Aged Care Act 1997 and the Aged Care Principles. If there are any matters not fully clarified within the content of the Guidelines, assistance should be sought from the Department of Health and Ageing or the state or territory health departments, as the Approved Providers of Transition Care. Independent legal advice should be sought as relevant on any particular matter contained within the Guidelines.
