Appendix 10: Improvement Plan

Purpose: This document is used by the service provider to detail their planned improvements as identified through the quality review process and through their own organisational improvement processes. The improvement plan is used by the quality review team to determine that the service provider is addressing any required improvements identified in the on-site visit and implementing ongoing improvements.

Instructions: The service provider completes the improvement plan following the on-site visit and:

  • Includes all of the required improvements and improvement opportunities in the improvement plan and any outstanding improvements identified in the self-assessment (that have not been completed) and returns it to the quality review team within 10 working days after receipt of the quality review report
  • Prioritises the improvements and allocates time frames for their completion; the quality review team may provide you with expected time frames for completion for required improvements, if deemed appropriate.
  • The improvement plan is used on an ongoing basis to demonstrate the completion of improvements in your organisation to the relevant quality review area.
  • You will be required to submit an updated improvement plan annually and will be contacted 4 weeks prior to this update being due.
The quality review team:
  • Reviews the submitted improvement plan to ensure that it addresses the required improvements and time frames identified during the on-site visit
  • Provides feedback to the service provider on their review of the improvement plan
  • Maintains a copy of the improvement plan.
