SSBA Reporting Forms

The SSBA Reporting Forms allow entities and facilities to report to the Department of Health details of reportable events in relation to the handling of SSBAs.

The first report (Initial Registration or Non-Registered Facility Report) is required to be submitted by a paper-based report and must be sent to the Department by Australia Post’s registered post service or courier. The addresses for postal reports can be found under the Contact SSBA Regulatory Scheme section.

Application for Extension

Use this form to apply for an extension when you are required to handle an SSBA for longer than two business days. Please note that this is not required when waiting for the results of confirmatory testing.

Change of Responsible Officer Details

Use this form to report changes to the Responsible or Deputy Responsible Officer or their contact details.

Change to Entity and Facility Details

Use this form to report changes to your entity or facility details.

Change to the Purpose for Handling an SSBA

Use this form to report changes to the purpose for handling a registered SSBA, including ceasing a purpose and adding a purpose.

Destruction

Use this form to report when the entire holding of an SSBA is destroyed or if the SSBA is a toxin and the remaining quantity falls below the reportable quantity. There is also a section in this form for applying to de-register. You do not have to fill in this form if you are a non-registered facility and have already provided this information in the Non-Registered Facility Report for Confirmed and Suspected SSBAs.

Incident Report

Use this form if you are a Registered facility and you need to report an incident that is defined as a reportable event, whether it occurs intentionally or by accident.

Initial Registration

Use this form if you wish to handle SSBAs. You must register within two business days of starting to handle the SSBA/s.

Non-Registered Facility Report for Suspected SSBAs and Confirmatory Testing Results

Use this form if you are Not Registered to handle SSBAs and you:

  • transfer a suspected SSBA for confirmatory testing; or
  • destroy a suspected SSBA prior to confirmatory testing; or
  • performed your own confirmatory testing on a suspected SSBA and the result confirmed the presence of an SSBA; or
  • sent a suspected SSBA to another facility for confirmatory testing and you are reporting the results of the test (positive or negative).

Non-Registered Facility Report for Temporary Handling and Disposal of an SSBA

Use this form to report the temporary handling and disposal of a known SSBA (previously confirmed in another facility). Temporary handlings must be for 7 working days or less.

Registration of a Linked Storage Unit

Use this form if you are a Registered facility and want to register a linked storage unit to store Tier 2 SSBAs under Part 4A.5 of the SSBA Standards.

This form can only be used if your facility is already registered to handle the SSBA. If you need to register a new facility please complete the Initial Registration form. If you need to register a New SSBA please complete the Start to Handle a New SSBA form.

Registered Facility Report for Suspected SSBAs and Confirmatory Results

Use this form if you are Registered to handle SSBAs and you:

  • undertake confirmatory testing on an SSBA you are not registered for and are reporting the results (positive or negative); or
  • transfer a suspected SSBA that you are not currently registered to handle for confirmatory testing; or
  • destroy a suspected SSBA that you are not currently registered to handle prior to confirmatory testing; or
  • are reporting an unsuccessful transfer or loss of a suspected SSBA.

Registered Facility Report for Temporary Handling and Disposal of an SSBA

Use this form to report the temporary handling and disposal of a known SSBA (previously confirmed in another facility) that you are not registered for. Temporary handlings must be for 7 working days or less.

Removal of Salmonella Typhi and Vibrio cholerae

Use this form if for the Removal of Salmonella Typhi and Vibrio cholerae.

Start to Handle a New SSBA

Use this form if you are a Registered facility and wish to report that you are handling a new SSBA that you are not currently registered to handle.

Transfers Between Facilities of the Same Entity

Use this form if you are a Registered facility and you are transferring SSBAs to another facility within the same entity.

Transfer In

Use this form to report the transfer in of SSBAs.

Transfer Out

Use this form to report the transfer out of SSBAs. There is also a section in this form for applying to de-register.
