Quality Use of Pathology
Investigation into the Reasons for Incorrect or Incomplete Pathology Request Forms - Royal North Shore Hospital - June 2008
The project studied the problem of incomplete or incorrect pathology request forms in order to identify the type of errors, their frequency and the reasons for their occurrence. The project was undertaken at Royal North Shore Hospital, a major teaching hospital, using the Area Health Service (Pacific Laboratory Medicine Services, PaLMS) pathology request form.

Help with accessing large documents

Help with accessing PDF documents
To view PDF (Portable Document Format) documents, you will need to have a PDF reader installed on your computer. A number of PDF readers are available through the Australian Government Information Management Office (AGIMO) Web Guide website.
