Public Health and Chronic Disease Grant Program

The Population Health and Sport Division is responsible for the Public Health and Chronic Disease Program, which commenced on 1 July 2016.

This Program consolidated funding and activities from four flexible funds:

  • the Health Surveillance Fund;
  • the Chronic Disease Prevention and Service Improvement Fund;
  • the Health System Capacity Development Fund; and
  • the Health and Social Surveys Fund.
The Program comprises the following themes:
  • Public Health and Chronic Disease Procurement
  • Cancer Control
  • Chronic Conditions Prevention and Management
  • Family Planning
  • National Palliative Care Projects
  • Population Health Improvements
The Guidelines for the Program provide an overview of the arrangements for the administration of, and activities that may be funded under, the Program.
