Sanctions Applied:
Revocation of approved provider status, unless an adviser, with a clinical background and nursing experience, who has the appropriate skills, qualifications and background to assist the approved provider to comply with their responsibilities, and is approved by the Commonwealth, is appointed by the approved provider for a period of six (6) months.

The approved provider is not eligible to receive Australian Government subsidies for any new care recipients until seven (7) days after the commencement of the adviser.

Revocation of approved provider status, unless the approved provider agrees to provide relevant training within six (6) months, at its expense, for its officers, employees and agents to support them in meeting the needs of care recipients.

Reason for Sanction:
The Accreditation Agency provided the Department with a report that identified serious risk at the Service.

The Department identified serious issues of concern, including:

*Care staff do not have adequate skills to perform their roles;
*Medications are not being provided as prescribed;
*Unskilled care staff are making decisions on the use of "as required" medications;
*Resident's nursing care has been provided without individual care plans to guide staff; and
*Methods for checking safe medication management have not identified unsafe staff practices.


Current Status of Service:
The Department and the Agency are continuing to monitor the Service.

The Residents and Representatives meeting was held on 8 November 2012.

A Nurse Adviser has commenced work at the Service.

On 13 November 2012, the Accreditation Agency made a decision that serious risk at the Service had been resolved.
