Information systems not supporting appropriate care planning and identification of care recipients’ needs, particularly in relation to skin care;

Initial and ongoing assessments of care recipients’ clinical care needs are not consistently conducted and/or documented;

Care recipients are not consistently receiving clinical care in accordance with their plan of care; and

Monitoring of skin integrity and associated clinical care has not been effective.


Current Status of Service:
The Department and the Agency are continuing to monitor the service.

Resident and Representative meeting was held on Wednesday 26 October 2011.

Nurse Adviser has commenced at the Service.

Sanction 2 expired on 17 November 2011.

Sanctions 1 and 3 expired on 17 April 2012.

Care recipients' specialised nursing care needs not being met;

Care recipients' pain management strategies not consistently implemented;

Care recipients' nutrition and hydration requirements not being provided;

Wound management not being attended to as required; and

Care recipients' continence needs not being met, impacting on their comfort and dignity.


Current Status of Service:
The Department and the Agency are continuing to monitor the service.

Resident and Representative meeting was held on Monday 31 October 2011.

On 9 November 2011 the Accreditation Agency found that the serious risk had been resolved.

Administrator and Nurse Adviser have commenced at the Home.

The sanctions imposed on Mornington Island Aged Persons Hostel do not apply to the new Approved Provider.

Sanctions remain with Mornington Shire Council.
