Consumer resources
Consumer resources promote Lifescripts within the waiting room, providing some initial information to those who may be considering a change to their lifestyle.The consumer resources include posters for practice waiting rooms, brochures and checklists to inform patients about the initiative and provide them with an opportunity to indicate to their GP if they wish to discuss lifestyle issues.
Practitioner resources
Practitioner resources include the Lifescripts assessment and prescription pads. General practices can obtain four different assessment and prescription pads that address quitting smoking, increasing physical activity, eating a healthier diet and maintaining healthy weight, and reducing alcohol consumption. Lifestyle advice can be tailored to each patient's needs or patients can be referred to other services, where available, to further support healthy lifestyle choices.- Assessment and prescription pads *

Help with accessing large documents

Help with accessing PDF documents
To view PDF (Portable Document Format) documents, you will need to have a PDF reader installed on your computer. A number of PDF readers are available through the Australian Government Information Management Office (AGIMO) Web Guide website.
