The Client Rights and Responsibilities Poster outlines the clients’ rights to be treated with respect, to be informed about treatment options, to be given a choice of hearing devices from Free-to-Client to Top-Up devices and privacy issues. It also outlines clients’ responsibilities to disclose relevant medical information, respect staff and participate in follow up care.The Contracted Service Provider Contract requires that the poster be displayed at each Permanent Site.
There is an online ordering facility for posters and client application forms.
Manual Claim for Payment Form
The manual claim for payment for is used when you are unable to make an electronic claim, this can occur for a variety of reasons, ie additional information, non standard request etc. We have specified below when the manual claim for payment form is required. Please ensure that you fill in the PDF Version of the Manual Claim for Payment form (PDF 105 KB) and the PDF Version of the Manual Claim for Payment form attachment. Sometimes a client may have a clinical need for a remote control due to a disability that makes it too difficult for them to manage the controls of their hearing device.In these cases, the Office can pay for the remote control for the client, if certain conditions are met.

Spare Aid Form
A number of clients can only wear a hearing device in one ear. If the hearing device is lost or requires repair, some clients are severely disadvantaged and have a great deal of trouble hearing without any assistance.For these clients, a spare aid may be fitted, if certain conditions are met.
The PDF Version of Spare Aid Guide Form (PDF 48 KB) or Spare Aid Guide Form (Word 330 KB) should be filled in and sent to the Office with a PDF Version of the Manual Claim for Payment form (PDF 105 KB) .
Non-standard device request form
The Office has approved a wide range of hearing devices to meet the needs of clients. However, a very small group of clients may need something from outside this group of approved devices.For these clients, a Request to Fit a Non-Standard Device (PDF 56 KB) can be completed and submitted to the Office for assessment.

Some clients have more severe communication difficulties. This can be because all sounds have to be very loud before they can be heard, or speech is not clearly understood even with a hearing aid, or they have another disability which makes it harder to communicate with people. Clients are able to choose to receive additional specialist hearing services from specialist audiologists at Australian Hearing.Contracted Service Providers must tell the Office that clients meet these criteria, by sending an OHS Complex Client Notification (Private Provider) Form (PDF 46 KB). Request for new voucher when existing has not expired
If a clients' hearing or health changes significantly after they have received the full entitlement of their current voucher (ie they have been assessed, received a hearing device and /or rehabilitation), then you are able to request a new voucher before the existing one expires.Where a client may need this, a should be completed with supporting evidence and sent to OHS Queensland. Eligibility Criteria for Refitting
Clients and Contracted Service Providers make joint decisions about the hearing devices that meet the client’s needs and lifestyle. Over time, a client’s hearing and circumstances may change so that new hearing devices are needed. Practitioners will exercise their clinical judgment when making decisions about refitting. Eligibility for Refitting outlines the information a practitioner has to consider when deciding to refit a client.Minimum Hearing Loss Threshold (MHLT)
Not all clients who have a hearing assessment want to be fitted with hearing aids, and many people who are tested have hearing that is almost normal. For people who have a very mild hearing loss, the Government has set certain rules that must be met before fitting or refitting hearing devices.
