Eligibility Confirmation Form (PDF 117 KB)
If you are unsure of your eligibility to receive hearing services under the Program, please download and complete the eligibility confirmation form and return it to the Office (address above). Alternately you can call the Client Contact Line on 1800 500 726.

DVA Maintenance Eligibility Form (PDF 68 KB)
If you are eligible to receive an exemption for maintenance agreement payments under the Department of Veterans Affairs, please download and complete the DVA Maintenance Eligibility Form and return to the above address.

Statutory Declaration for Replacing Lost Aids (PDF 17 KB)
If you have lost your hearing device or it is damaged beyond repair, please download and complete the statutory declaration form outlining your circumstances. This form should be given to your hearing practitioner.

Voucher Verification Request Form (PDF 187 KB)
This request form should be completed at the time of assessment.

Clients Rights and Responsibilities Poster (PDF 1114 KB)
The Client Rights and Responsibilities Poster outlines the clients’ rights to be treated with respect, to be informed about treatment options, to be given a choice of hearing devices from Free-to-Client or Top-Up devices and privacy issues. It also outlines clients’ responsibilities to disclose relevant medical information, respect staff and participate in follow up care.

Help with accessing large documents

When accessing large documents (over 500 KB in size), it is recommended that the following procedure be used:

  1. Click the link with the RIGHT mouse button
  2. Choose "Save Target As.../Save Link As..." depending on your browser
  3. Select an appropriate folder on a local drive to place the downloaded file

Attempting to open large documents within the browser window (by left-clicking) may inhibit your ability to continue browsing while the document is opening and/or lead to system problems.

Help with accessing PDF documents

To view PDF (Portable Document Format) documents, you will need to have a PDF reader installed on your computer. A number of PDF readers are available through the Australian Government Information Management Office (AGIMO) Web Guide website.
