Application and Assessment Guidelines
Following the 2009-10 Review of the Stoma Appliance Scheme (SAS), the Australian Government agreed to introduce a new operating framework for the Scheme.
The SAS Application and Assessment Guidelines have been developed by the Department, with input from the Stoma Product Assessment Panel (SPAP) to explain the revised application and assessment processes for the Scheme.
These guidelines will continue to be revised when required to ensure that they remain contemporary and accurate and provide the information necessary to prepare high quality applications.

Lodging an Application
The SPAP Secretariat manages application lodgements. Applications should be completed using the electronic form(s) below and mailed to:

To complete, print and save copies of the application form you will need Adobe Reader 9.0 (or later) software installed on your computer. You can download Adobe Reader or upgrade your existing version for free at: www.adobe.com. Earlier versions of Adobe Reader will not allow you to save copies of the application form.

A checklist is provided at the end of each Application Form to prompt applicants to check that they have included all relevant and requested information.

Help with accessing large documents
When accessing large documents (over 500 KB in size), it is recommended that the following procedure be used:
- Click the link with the RIGHT mouse button
- Choose "Save Target As.../Save Link As..." depending on your browser
- Select an appropriate folder on a local drive to place the downloaded file
Attempting to open large documents within the browser window (by left-clicking)
may inhibit your ability to continue browsing while the document is
opening and/or lead to system problems.
Help with accessing PDF documents
To view PDF (Portable Document Format) documents, you will need to have a PDF reader installed on your computer. A number of PDF readers are available through the Australian Government Information Management Office (AGIMO) Web Guide website.
