Pathways Home
(Schedule B of the Australian Health Care Agreements)

Pathways Home is a new program that the Australian Government is funding under the 2003-08 Australian Health Care Agreements. Over five years, the Australian Government will provide $253 million to the states and territories to increase the rehabilitation and stepdown (convalescent) services provided to patients that are leaving hospital, particularly those patients who are older or who have some form of mental disability. This will mean patients get that bit of extra help they need to get back on their feet.

The funding for this program forms part of the Australian Government's $42 billion contribution to public hospitals over the five years of the Australian Health Care Agreements. To access Pathways Home funding, each state and territory was required to submit a plan of their proposed expenditure by 31 December 2003.
