As part of the Living Longer Living Better aged care reform package¸ the Department of Health and Ageing is seeking applications from a wide range of non-government and government organisations other than the Commonwealth for a grant to deliver innovative specialist and palliative care and advance care planning advisory services for aged care providers and GPs caring for recipients of aged care services.
