Attachment 2: Statement of Income and Expenditure of Commonwealth Grant Funds

Instructions

  • Statements of Income and Expenditure are required as part of each Progress Report. Submission dates are specified in the Schedule of the Funding Agreement (the Schedule).
  • The Statement of Income and Expenditure should only record activity against the Budget line items listed under Item B of the Schedule, and should only show expenditure of Commonwealth grant funds.
  • Accrual based accounting principles should be used when reporting on spending activities.
  • Report all income and invoiced expenditure.
  • Fields requiring Participant input are shaded. Populate the shaded fields only.
  • Ensure the Certificate of Compliance is signed by two officers authorised in accordance with clause 8 of the Funding Agreement.
  • Audited Statements of Income and Expenditure must be submitted for the total funding period with the Final (third) Report. Audited Statements of Income and Expenditure must include:
  • A Scope and an Audit Opinion signed by an approved auditor certifying that the financial accounts are complete and accurate as per clauses 11.3(f) and 11.4(d) of the Funding Agreement.
  • A declaration by the Participant.
  • Variations between the Budget item amounts agreed in Item B of the Schedule and actual expenses against those items will be monitored by the Department. Any variations should be explained under question 13 of the Progress Report.
Please Note:The requirement to have Statements of Income and Expenditure of Commonwealth funds audited separately from your organisation's other auditing activity may result in an additional cost being incurred by your organisation. Any such costs will need to be met by your organisation.

Certificate of Compliance

We, the undersigned, certify for the abovementioned Project that:

1. all Funds received have been expended for the purposes for which they were provided and in accordance with the Agreement;

2. all Funds have been accounted for;

3. all salaries and allowances paid to persons involved in the Project are in accordance with applicable awards or agreements in force; and

4. That all obligations in respect of GST have been complied with.

We acknowledge that it is an offence under Section 137 of the Criminal Code Act 1995 to provide false or misleading information or documents to the Commonwealth.
