Information for pharmacies selected for the MedsCheck / Diabetes MedsCheck Pilot.

This page is for the use of Pharmacies who have been selected to participate in the MedsCheck / Diabetes MedsCheck Pilot.

You will have received written confirmation of your participation, a CD containing all necessary software and user guides to set your system up, and have been allocated a Pilot Registration Code.

If you have downloaded the SmartForm Signing Utility (SSU) and set up an AUSkey, including where applicable, an AUSkey for each pharmacist who will be performing reviews, you may click the link below to download the Assessment Tool.

NB: Remember, a new form MUST be downloaded for each review undertaken. Each form downloaded can only be submitted once for payment.
