The type 2 diabetes risk evaluation supports general practitioners in addressing the health needs of patients 40 to 49 years of age who are at high risk of developing type 2 diabetes.

It enables general practitioners to review a patient's risk factors and, if appropriate, to refer the patient to a subsidised lifestyle modification program to assist with the prevention of type 2 diabetes.
The intention of a lifestyle modification program is to help people modify their risk factors to delay or prevent the onset of type 2 diabetes.
Eligible patients may also be referred to a lifestyle modification program following a health assessment for:
- people aged 45 to 49 years who are at risk of developing chronic disease or
- Aboriginal and Torres Strait Islander people (MBS item 715)
