Reason for Sanction: The Agency identified serious risk, in relation to Standard 1, Item 1.6: Human Resource Management; Item 1.8: Information Systems; Standard 2, Item 2.4: Clinical Care; Item 2,10 Nutrition and Hydration; Standard 3, Item 3.4: Emotional Support; Item 3.5: Independence; Item 3.7: Leisure Interests and Activities; and Item 3.8 Cultural and Spiritual Life.
Current Status of Service: The Department and the Aged Care Standards and the Accreditation Agency are continuing to monitor the service.

The Service has appointed an adviser.

On 13 February 2009 the Agency advised that serious risk had been mitigated
