Including: Inappropriate clinical care; lack of suitably qualified and skilled staff; poorly maintained and inadequate food processing equipment; increased risks of infection because of poorly maintained laundry and cleaning equipment; lack of effective call system impacting care provision and resident's safety.

Current Status of Service
Service was transferred to a new approved provider on 14 August 2009

Sanctions do not apply to the new approved provider.
