On 9 August 2004, the Approved Provider applied to have the sanctions lifted.
On 31 August 2004, the Agency made a decision to accredit the service for a further 2 year period from 5 October 2004 to 5 October 2006 following a site audit on 3-5 August 2004.
On 6 September 2004, the Department lifted sanctions.

Approval as an Approved Provider of aged care services revoked unless a nurse adviser is appointed remains in place.
The Approved Provider has submitted an application to transfer the places to another provider.
The application to transfer the places to another provider has been approved by the Department.
The nursing home currently has no residents.
New Approved Provider on 24 August 2001, Presbyterial Aged Care.

The Department and the Aged Care Standards and Accreditation Agency are continuing to monitor the Service.
The Approved Provider has appointed a nurse adviser.
The Agency made a decision on 4 February 2005 to revoke the Service’s accreditation effective 22 April 2005 and advised that the serious risk had been resolved.
On 18 February 2005 the Approved Provider applied to the Agency for a reconsideration of the accreditation decision.
The Approved Provider advised the Department on 15 April 2005 that the Service ceased to operate from 14 March 2005.
