Current Status of Service: The Aged Care Standards Accreditation Agency reports that no serious risk was identified at the time of its support visit. The Approved Provider applied for the sanctions to be lifted.

The sanctions were lifted on 31 October 2000.

On 2 December 2005, the Agency advised that the serious risk has been mitigated.

Sanction 2 (no funding for new residents) expired on 21 January 2006.

On 16 February 2006 the Agency advised that the home is now fully compliant with the Accreditation Standards.

On 21 April 2006, Sanction 1 (appointment of nurse adviser) expired.

The service has appointed an adviser.

Sanctions expired 7 June.

Sanctions Applied: No Commonwealth funding for new residents for a period six (6) months.
Date Sanction Imposed: 21 October 2004
Reason for Sanction: The sanction was imposed due to a breach of the Accountability Principles under the Aged Care Act 1997 where the Approved Provider failed to allow the Aged Care Standards and Accreditation Agency (ACSAA) access to the home in order to undertake its monitoring role.
Current Status of Service: OThe Agency has recommenced visits to the home. The Department continues to monitor the service.

On 23 November 2004 the approved provider applied to have the sanction lifted. Following consideration of the information in the application the Department has not lifted the sanction.

On 15 December 2004 the approved provider re-applied to have the sanction lifted. The Delegate is considering the application. Following consideration of the information in the application the Department has not lifted the sanction.

On 10 March 2005 the approved provider made further application to have the sanction lifted.

On 15 March 2005, the Delegate considered the information and decided to lift the sanction.

Note: As of 2 October 2007 all Australian Government subsidised residential care places at the service were transferred to a different approved provider. The sanctions imposed on Montbello Pty Ltd do not apply to the new approved provider.

Sanctions lifted on 25 October 2007.

All residents were transferred to other facilities and the home was closed by 26 October 2007.

On 4 January 2008, the Agency advised that serious risk in relation to expected outcome 2.13 Behaviour management had been mitigated.

Sanction 1 - No Australian Government funding for new residents expired on 11 June 2008.

Sanction 2 expired 13 June 2008.

Current Status of Service: The Approved Provider applied to have the sanctions lifted. The sanction revoking Approved Provider status unless a (nurse) adviser is appointed was varied on 22 March 2001.

The sanction providing that no additional aged care places will be allocated to the provider for a period of twelve months was lifted on 22 March 2001.
Sanction 2 Applied: The Approved Provider must retain the (nurse) adviser until the day after the Agency makes its 2001 accreditation decision.

The Department found there was an immediate and severe risk to the safety, health or well-being of residents.
Current Status of Service: The Approved Provider nominated an (aged care nurse) adviser.

The Department approved the nominated (nurse) adviser.

The Approved Provider held a meeting to inform residents, relatives and other stakeholder about the situation.The Aged Care Standards and Accreditation Agency notified the Department that the risk to residents no longer exists.

The Agency has accredited the facility for 1 year (15/12/2000 to 15/12/2001).

The Accreditation Agency will monitor the facility's improvement plan on a regular basis.

The Approved Provider applied to have the sanctions lifted.

The sanction revoking Approved Provider status unless a (nurse) adviser is appointed was varied on 22 March 2001 so that the approved provider must retain the (nurse) adviser until the day after the Agency makes its 2001 accreditation decision.

Current Status of Service: The Approved Provider applied to have the sanctions lifted.

The sanction restricting commonwealth funding for new residents for a period of 6 months was lifted on 1 August 2001.
Sanction 2 Applied: Approval as an Approved Provider of aged care services revoked unless an adviser is appointed for a period of 6 months.

Reason for Sanction: The Approved provider did not comply with all its responsibilities in relation to Accreditation Standards.

The Department found there was a risk to the safety, health or well-being of residents.
Current Status of Service: The Approved Provider nominated an adviser (with nursing and aged care expertise).

The Department approved the nominated adviser.

The Approved Provider held a meeting to inform residents, relatives and other stakeholder about the situation.

The Accreditation Agency has varied the homes period of accreditation from 3 years to 2 years and 1 month. The homes accreditation will now expire on 24 December 2001.

The Accreditation Agency and the Department are closely monitoring the situation at the home.

The Aged Care Standards and Accreditation Agency notified the Department that the serious risk to residents no longer exists.

The sanction restricting Commonwealth funding for new residents for a period of 6 months was lifted on 1 August 2001.

The sanction expired on 17 November 2001.

The Department found there was an immediate and severe risk to the safety, health or well-being of residents.
Current Status of Service: The Approved Provider held a meeting to inform residents, relatives and other stakeholders about the situation.

The Approved Provider nominated (nurse) advisers and an administrator. The Department approved the nominated nurse advisers and administrator.

The Agency notified the Department that the risk to residents no longer exists. The Approved Provider applied for reconsideration of its accreditation status.

The Agency reconsidered the accreditation status and decided that the facility will be accredited until 9 January 2003.

The Accreditation Agency will monitor the facility's improvement plan on a regular basis.

The Approved Provider applied for the lifting of sanctions.

The Sanctions were lifted on 10 January 2001.

Facility: Westminster Village Nursing Home

The Agency accredited the facility for 9 months (19/12/2000 to 19/09/2001).

The Approved Provider held a meeting to inform residents and relatives about the changes.
