Quality Reporting Online System

This online system has been developed to enable Service Providers to submit their Quality Reporting Self Assessment and Improvement Plans securely online.

Getting Started

You will be notified by the Department of Health and Ageing when your service outlet is due to commence the Quality Review process.

The Self Assessment Notification letter sent to the service outlet under review includes: a “Request for Access: Quality Reporting Online” form. Please complete and return this form to fax number: 02 6289 1504 or via email to: qr@health.gov.au

Once you have submitted the Request for Access form, you should then receive an email titled “Quality Reporting – Welcome to Quality Reporting On Line”. This email contains a user name and password along with further details about how to log on and submit your Self Assessment and Improvement Plans online.

Further information on submitting your Self Assessment and Improvement Plans is contained within the “Service Provider User Guide: Quality Reporting Online” guide which is an attachment to the “Quality Reporting – Welcome to Quality Reporting On Line” email.

If you have not received this form click on the Contact Us for details relevant to your State or Territory.

If you have any problems accessing or using the Quality Reporting Online System contact the Quality Reporting Team at the Department of Health and Ageing, via email at qr@health.gov.au or phone (02) 6289 7110.

Logging In

  • Once you have received an email containing your user name and password click the link in the email to login (https://external.health.gov.au).
  • Enter your username and password and click the Login button.
  • The Quality Reporting home page will appear.
Top of pageStep 1: Open the Review
  • Under the “Home” link on the left hand side of the screen, click on “Reviews”
The Review List screen will show a list of all the reviews to be completed, as well as those that have already been completed. Any reviews with a status of Self Assessment Tool Created or Improvement Plan Created are waiting for your completion.
  • Click on the name of the review you wish to open.
  • This action will open into the specific review for the designated service under review.
  • This view will show individual tables for the Self Assessment Tool, Improvement Plan and all letters sent by the Department in relation to this specific review.
Note: It is possible to type directly into the Self Assessment Tool or Improvement Plan.

Alternatively, Microsoft WORD versions of the Self Assessment Tool and Improvement Plan are available under the Community Care Common Standards . Once you have completed your responses within the WORD version, it will be necessary to copy and paste responses for individual questions into the electronic document found on this website prior to submission.

Step 2: Completing the Self Assessment Tool or Improvement Plan
The sections within the Self Assessment Tool or Improvement Plan may be completed in any order.
Changes can be made to each section until the report is submitted (see Step 3 below).
All sections must be completed and finalised before the report can be submitted.

Self Assessment Tool

  • Under the Self Assessment link, click on the relevant section for completion:
  • Service Details
    - Standard 1
    - Standard 2
    - Standard 3
  • Declaration
Fill in the information as prompted under each section and click the “Save” button to save data. Once completed click on the “Finalise” button at the bottom right hand side of the screen.

Note: Each Expected Outcome (EO) within Standards 1, 2 and 3 must be completed and finalised before the Self Assessment Tool can be submitted.

Improvement Plan

  • Under the Improvement Plan link, click the individual identified improvements for completion
  • Click the “Edit” button and complete the sections as requested for each individual improvement
  • Click the Save button
  • Prior to submission, each individual improvement needs to be finalised by clicking on the “Finalise” button
Step 3: Submitting the Quality Report
Once you have completed the Quality Report it must be submitted.
  • Once you have completed the Self Assessment or Improvement Plan it must be submitted via the Quality Reporting Online System.
  • Ensure each individual section has been finalised
  • Click the “Submit” button
