Community Partners Program (CPP)

The Community Partners Program (CPP) aims to promote and facilitate increased and sustained access to aged care support services by culturally and linguistically diverse communities with significant aged care needs.

The Community Partners Program (CPP) aims to promote and facilitate increased and sustained access to aged care support services by culturally and linguistically diverse communities with significant aged care needs.

The primary outcomes of the CPP will include:

    • More aged care support services delivering culturally appropriate care to older persons from culturally and linguistically diverse communities;
    • Older people from culturally and linguistically diverse communities having increased access to culturally appropriate age care support services;
    • Older people from culturally and linguistically diverse communities having greater capacity to make informed decisions about aged care support services; and
    • Formalised arrangement made that will sustain the benefits of the Project and its strategies and continue to promote and facilitate increased access by culturally and linguistically diverse communities to aged care support services.
Guidelines for the CPP program have been drafted to assist in the development and management of CPP funded projects. They provide details of the program, eligibility, accountability and roles and responsibilities and are available for download in HTML or PDF format:

Community Partners Program 2008-2009

The Community Partners Program (CPP) provides grants to community based organisations representing culturally and linguistically diverse communities. These organisations use these grants to provide support for older people from culturally and linguistically diverse communities in accessing aged care services.

This project will work with Palliative Care service providers in the Inner West aged care planning region. The organisation will provide cultural briefing to hospital staff, allied health workers and residential care staff offering Palliative care services. In addition, information sessions will be conducted to enhance the awareness of the Chinese community “what Palliative Care is “and how individuals and carers can work together and support each other. These findings will be documented as a resource for service providers.
In addition, the organisation in partnership with the Australian Korean Welfare Organisation will form local working groups to gather information available on Medication management, discuss /identify issues with Korean, Chinese and Vietnamese communities about their understanding of Medication management, work with local GPs (Chinese) and local pharmacist and community nurses and develop a FACT sheet on medication management. These Resource Materials will be translated into Korean, Chinese and Vietnamese. The Fact Sheets will be widely distributed to the 70+ population to enhance awareness of medication management amongst the elderly and the risks involved in mismanagement of medication.

This project will promote awareness of, and facilitate increased access to aged care services to the Ukrainian community in the Illawarra region. Links with Aged Care Providers in the Illawarra will be strengthened to facilitate culturally appropriate care. Key CPP activities will include developing links between the Ukrainian community and the aged care sector, eg, visits to residential aged care facilities.
Indonesian Welfare Association IncNorthern Sydney and Western Sydney aged care planning regionsIndonesianCore CPP activities. This project will promote awareness of, and facilitate increased access to aged care services to the Indonesian community in the Northern Sydney and Western Sydney regions. Links with Aged Care Providers in the regions will be strengthened to facilitate culturally appropriate care. Key CPP activities will include continuing partnership with Uniting Care Northern Sydney, especially in development of Indonesian cluster in Ermington and continue to build links with other services, eg, Aged Care Assessment Teams, New South Wales Transcultural Aged Care Service PICAC.

The organisation will identify current service providers in Wollongong area. Research available resources or relevant materials in Italian and discuss current practices with service providers. In partnership with palliative service providers, the organisation aims to develop plan of action that includes identification of cultural specific issues, normal practices, how to involve families/carers/ identification of cross cultural training undertaken by palliative care staff and document its findings as a resource material for palliative service providers.

The organisation will work with Palliative care service providers, including Westmead and North Shore Hospitals to discuss and highlight Palliative Care issues relating to minority emerging groups, such as Afghan and Iranian communities from Muslim background and develop a ‘Best Practice Palliative Care Model’. The process includes documenting issues and current practices, collating feedback from the identified communities, conducting focus groups and developing ‘A Resource’ in partnership with the Hospital staff responsible for Palliative care. This plain English Resource material will be translated in Dari and Farsi languages.

The organisation will form a working group consisting of Local GPs, Pharmacists, allied Health Workers and Community Care providers (CACP) to develop a culturally appropriate Medimate Information Resource, which will be piloted with the Spanish speaking community that includes feedback from focus groups. The final resource will be compiled in English and then translated into Spanish. The working group will consider relevant material available, will seek feedback from expertise, conduct focus groups to gather information and identify issues/barriers for the development of the Resource material.

Diversitat City of Greater Geelong, Borough of QueenscliffeItalian, Dutch, German, Croatian, Greek, Polish, Macedonian, Serbian, Ukranian, Hungarian, SpanishThe program is looking at working with Aged Care Assessment Teams and CALD assessment recipients to develop a clear and easy process following assessment to access either package providers or residential providers. The research they have done shows CALD clients are confused after such assessments and in need of some support.
Federation of Indian Association of Victoria IncMetropolitan MelbourneIndianThis project will continue to produce CALD brochures and provide information sessions regarding the barriers to Indians accessing aged care and set up networks with GPs to provide appropriate care to the elderly. The extension of their project will be actioned by focusing on working with Carers Victoria and the Alzheimer’s Association to gain culturally appropriate care for their elderly by the promotion of cultural issues relating to dementia. Will also be working to develop community education programs on mental health issues amongst Indian elderly people to reduce attached cultural barriers.
Filipino Community Council of Vic IncWestern, Northern, Southern and Eastern RegionsFilipinoThis project will develop links with Aged Care Assessment Teams, Carelink Centres, Alzheimer's Australia and the Cancer Council of Victoria. They are looking at expanding their area to pockets in the 4 Metro regions that have a large concentration of Filipinos. They are looking at developing a Palliative Practices about Filipino resource kit that includes old time music, appropriate religious representation etc.
Fronditha Care IncVictoria StatewideGreek speaking backgroundsThis project will add another feature to the website which will give detailed advice about death and dying issues to professionals dealing with Greek elderly in palliative care. The Project aims to enhance the ability of Greek elderly to play an active part in their medication management by providing them with advice, information and assistance which will increase their self confidence and skills in managing the task and by building links and partnerships with the formal service system (e.g. Division of General Practice) and existing community groups of Greek elders such as Elderly Citizens Clubs.

They are looking at working in partnership with Carers Victoria Respite Connections and senior citizens hostel to develop and distribute culturally appropriate information regarding respite services, promote respite to carers and provide caring training programs to carers. They will in turn develop relationships with these organisations. They are looking at increasing awareness, knowledge and support for carers in their community.
Macedonian Community Welfare Association IncNorth West, South East (Dandenong, Springvale) Metropolitan RegionsMacedonian speakingThis project will continue the delivery of cultural briefings and forums in conjunction with the Vic Partners in Culturally Appropriate Care as important educational and informative strategies. It will develop valuable resources for its community through educational and information sessions on cultural specific issues within the Macedonian community to residential and palliative care staff. Develop community awareness on 'Supporting a Person Who Needs a Palliative Care' for the Macedonian speaking community. Continue networking with appropriate organisations (eg General Practitioners, Pharmacy and Webster Care). Among others develop a booklet on coping with death and dying strategies for carers and translate the Medimate booklet into Macedonian language.
Maltese Community Council of Victoria IncNorth-West Metropolitan, Parts of South-East Metropolitan and La Trobe ValleyMalteseThis project will extend to the South East and La Trobe Valley areas and educate the elderly in the community about the transition to Residential Care and the psychological aspects for moving to Residential care. They will produce a resource booklet in Maltese about entering Residential Care. They are proposing to hold a workshop for Aged Care - Respite and Palliative care workers about the Maltese culture.

They will organise a Community Forum on Aged Care issues and negotiate protocols with service providers regarding the role of its Cultural Peer Facilitators.
Australian-Polish Community Services IncNorthern and Western Metropolitan Regions of VictoriaPolishThe new extended project will work on culturally appropriate palliative care. They will look at developing resources for the community and aged care providers. They will also be looking at developing relationships with Carers Victoria to adapt some of their Carer education sessions to CALD appropriate care.

It will also implement a database of community leaders and CALD clients who attended information sessions to keep them up to date.
St John of Kronstadt Russian Welfare Society IncMetropolitan MelbourneRussian speakingThe new extended project will work on developing culturally appropriate material on palliative care for aged care facilities and publish on web page. St John will produce a series of cards with essential words in English and Russian for better communication. Will compile Russian food recipes for aged care facilities.
Serbian Welfare Association of Victoria IncMetropolitan Melbourne, South Eastern Region of MelbourneSerbianThis project will expand services to the Serbian community to the Southern, Eastern and Western Regions and continue to target the South East. They will provide information sessions and invite guest speakers to present on various aged care topics including Respite Care, Dementia, Palliative Care, Community Aged Care Packages and Residential Care. They are proposing to develop a resource kit, organise site visits and work closely with Alzheimer's Australia.

They are increasing the amount of visits plus additional senior groups to improve awareness on residential and other aged care support services. Will continue to build appropriate networks.

This project provides CALD communities with structured training that supports them in understanding and accessing aged care services. Volunteers are provided with aged care skills and knowledge that better enable them to support aged care clients.
