Projects funded under this program aim to:

    • identify effective, economically viable and sustainable models of practice;
    • facilitate the growth of the aged care nurse practitioner workforce; and
    • improve access to primary health care for clients of residential and community aged care service.
An open application process was conducted and 31 organisations (conducting 32 projects) have now been approved for funding. The 32 funded projects were announced by the Minister for Mental Health and Ageing, the Hon Mark Butler on 15 December 2011.

Funded organisations comprise a good mix of models including individuals/sole traders, small aged care providers, peaks, Divisions of General Practice, Medicare Locals, consortiums and other organisations in the aged care sector. Models are being supported in all jurisdictions while also providing funding to services in rural, regional and remote areas with organisations that support the delivery of Aboriginal and Torres Strait Islander and culturally specific care.

The program is being supported by a national evaluation to measure and compare the extent to which the different models of practice meet the stated program aims above. The evaluation being led by the University of Canberra will collect a wide range of both qualitative and quantitative data and information on the service delivery models and practitioner and consumer experience.
