PHN Program Guidelines and Policies

The Primary Health Networks (PHN) grant program guidelines provide information on:

  • The PHN program
  • PHN establishment
  • Eligibility
  • Probity and
  • Governance and accountability.
There are additional annexures that specifically relate to the Primary Mental Health Care Activity and Drug and Alcohol Treatment Activity. These annexures supplement the PHN Grant Program Guidelines.

The guidelines and the annexures may be updated during the course of the PHN Program to reflect further details of the PHN Program as it evolves.

The Standard Funding Agreement for the PHN Program, which sets out the obligations relating to the funding for PHNs, continues to be the Department of Health’s superseded 2015 Standard Funding Agreement, which is provided below. Note for other program funding please refer to the current Commonwealth Standard Grant Agreement used by the Department of Health.

PHN Program Complaints Policy

The PHN Program Complaints Policy describes how the Department of Health resolves complaints about a PHN or the PHN Program.
