The Healthy Kids Check must include the provision of health advice and information to the patient's parent or carer, using the Get Set 4 Life – Habits for Healthy Kids guide and other relevant information such as a parent-held child record. The medical practitioner, practice nurse or Aboriginal and Torres Strait Islander health practitioner must note if the patient's parent or carer has been referred to this website to obtain a copy of the Guide, noting that hard copies are no longer available.

There is no requirement that the patient’s parent or carer must be provided with a copy of the guide.
