Contributors can complete the Federal Government 30% Rebate Application Form (PDF 17 KB) and send it to their health fund to receive the 30% Rebate.

Health Funds have the option of incorporating this form into their application form. However, they should contact the Department if they wish to pursue this option. Contact : 1800 020 103.

Health funds are encouraged to download and print this form as required. As this web site contains the most current version of the form, health funds may find it useful to make this site a link from their own web site so that members can print out the form themselves.
