Private Health Insurance Rebates
The Australian Government provides a 30% rebates on appropriate private health insurance cover
To order a copy of the Private Patients’ Hospital Charter in large print, braille or audio CD please send an email with your request to privatehealth@health.gov.au or phone (02) 6289 9853 (24 hour answering machine).
To claim the Private Health Insurance Rebates, members can complete the form available at Medicare Australia and send it to their health insurer. http://www.medicareaustralia.gov.au/public/forms.jsp
Private health insurers have the option of incorporating this form into their application form. However, they should contact the Department if they wish to pursue this option. Contact: 1800 020 103.
Private health insurers are encouraged to download and print this form as required. As the Medicare Australia website contains the most current version of the form, insurers may find it useful to include a link to the Medicare Australia website from their own website so that members can print or e-mail the form themselves.

Help with accessing large documents
