Healthy for Life Service Toolkit
Section 6: Analysing and Planning
The Healthy for Life Service Toolkit has been developed to assist primary health care services (organisations) undertake activities required to meet the aims of the Healthy for Life program, which are: to improve the health of Aboriginal and Torres Strait Islander mothers, babies and children; to enhance the quality of life of people with chronic illnesses; and over time, to reduce the incidence of adult chronic disease.
This section shows you how to bring together all the information gathered. It enables you to decide what is most important, set priorities and put them into an Action Plan for implementation.
A slide show aids in discussions regarding the development of Action Plans.
