Healthy for Life Service Toolkit
The Healthy for Life Service Toolkit was developed to assist primary health care services (organisations) undertake activities required to meet the aims of the Healthy for Life program, which are: to improve the health of Aboriginal and Torres Strait Islander mothers, babies and children; to enhance the quality of life of people with chronic illnesses; and over time, to reduce the incidence of adult chronic disease.
The Healthy for Life Service Toolkit contains a number of proformas and templates for use in program development and implementation.
